According the director, the center has water leak in the kitchen and it require a repair before they can do the lead water testing, which poses/posed a potential health, safety or personal rights risk to persons in care.
Disaster and Mass Casualty Plan — Based on observation, interview and record review, the licensee did not comply with the section cited above. According to the director, she is unable to find the disaster drill log, which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on observation,interview and record review, the licensee did not comply with the section cited above. During today's inspection, the director was unable to provide MMR shot record and the admit was unable to provide her immunication record, which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above. LPA observed her admit does not have manndated reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.
