Buildings and Grounds — Per Director roaches were observed in the kitchen area and classroom nearby, which was confirmed by two staff members, which posses potential health and safety risk to children in care. Additionally, LPA observed two dead roaches in the kitchen cabinet and one in the cabinet under the sink located in the 2A classroom. Cocroach traps and baking soda observed in the cabinets.
Mandated reporter training — Based on observations, records review and interview , the licensee did not comply with the section cited above. 6 out 9 staff files were missing Mandated Reporter Training , which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on observations , records review and interview , the licensee did not comply with the section cited above. Three out of nine records were missing proof of mmr and Tdp, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on five staff file review, the licensee did not comply with the section cited above in 3 out of 5 staff, Staff#1, staff#2, and staff#3 did not have Health screening form on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on 10 children files review, the licensee did not comply with the section cited above in 2 out of 10 children did not have Physical Report on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on sign in/out sheet review, the licensee did not comply with the section cited above, during this inspection 74 children were present and 63 children were signed in which poses/posed a potential health, safety or personal rights risk to persons in care.
