Personnel Requirements — Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. One staff's (S2) TB test result was unavailable during today's inspection.
Sign In and Sign Out — Based on sign in and out records reviewed from 08/07/2025 to 08/20/2025 and from 10/01/2025 to 10/07/2025, LPA observed several children were missing sign ins or sign outs by staff or authorized representatives. This poses a potential risk to the health, safety or personal rights risk to persons in care.
Criminal Record Clearance — Based on file review, LPA observed that staff, S8 has fingerprint clearances but the clearances have not been transferred to this facility. This poses a potential risk to the health, safety and personal rights of children in care.
Mandated reporter training — Upon file review, LPA observed that staff, S1 is missing the current Mandated Reporter Training. This poses a potential risk to the health, safety and personal rights of children in care.
Responsibility for Providing Care and Supervision — Based on interviews conducted it was determined there was inadequate supervision that is why incident was able to occur where child (C1) tided a jump rope around the neck of child (C2) and choke them. This poses a potential risk to the health and safety of children in care.
Teacher-Child Ratio — the Facility was operating out of ratio when one partially qualified teacher opened the Facility on the following dates: 05/14/24, 05/16/24, 05/21/24, 05/23/24 and was the only staff present with anywhere from one to six children present during the early morning session. This presents a potential risk to the health, safety, or personal rights of children in care.
Site visit notice posted at the facility — the Notice of Site Visit from the Department's last visit to the Facility on June 4, 2024 was not posted upon LPA's arrival to the Facility.
Criminal Record Clearance — Staff 2 (S2) & Staff 7 (S7) began working at the Facility on Monday June 10, 2024, but were not associated to the Facility until 06/11/2024 (S2) and June 12, 2024 (S7)
Buildings and Grounds — Based on observation, there was not a cover on the electrical outlet near the entrance door that has the stairs. This poses a safety hazard to children in care.
Responsibility for Providing Care and Supervision — Staff was not watching the children as it was a maintenance worker from the school district who saw the children and reported it to the Director.
Child's Records — C2's file had no admission paperwork in it.
Admission Policies — There was not a signed parents rights in C2's file.
