Criminal background check — Based on observation, interview and record review, the licensee did not comply with the section cited above in the licensee failed to associate the current Site Director Kimberly Banks, who was present and working at the facility as of 10/31/2023 which poses an immediate health, safety or personal rights risk to persons in care.

101516.5(b)(1) Teacher-Child Ratio (b) There shall be a staffing ratio of 1 teacher & 1 aide present to every 28 children... (1) A teacher shall supervise no more than 14 children or with an aide a maximum of 28 children. This regulation was not met as evidenced by: Based on the agency's self - reported Unusual Incident, on 11/04/2024 a facility staff was left to supervise 19 children alone, for approximately 15 minutes, 7:30 AM and 7:45 AM, which is a potential risk to the health & safety of children in care.
Child's Records — Based on interview and record review, the licensee did not comply with the section cited above in that three children records were not available for review or completed per Site Directors which poses a potential health, safety or personal rights risk to persons in care.
Disaster and Mass Casualty Plan — Based on interview and record review, the licensee did not comply with the section cited above in the director was unable to provide proof of documentation of disaster drills conducted which poses a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on observation, interview, the licensee did not comply with the section cited above in by the directors own admission the staff utilize 1 of the 2 restroom stalls with a toilet and sink, leaving the facility with only 1 toilet and 1 sink designated and/or available for school age children, which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in the director was unable to provide proof of measles and pertussis, which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 1 staff present did not renew the required mandated reporter training after 12/2023 which poses a potential health, safety or personal rights risk to persons in care.
Child Care Center Director Qualifications and Duties — Based on interview, record review, the licensee did not comply with the section cited above in the licensee failed to ensure a director or staff complete 16 hours of health and safety training if necessary pursuant to Health and Safety Code Section 1596.866., which poses a potential health, safety or personal rights risk to persons in care.
