Personal Rights — Two children disclosed that their ears were pulled by an adult. A parent reported their child stated he/she was getting pinched by staff and a video showed an adult grabbing a toddler's upper arm to redirect the child. This is an immediate risk to the personal rights and health and safety of children in care.
Criminal background check — Based on record review, the licensee did not comply with the section cited above as LPA observed an individual, who does not have criminal record clearance, was present and caring for children during today's visit which poses an immediate health, safety or personal rights risk to persons in care.
Personal Rights — Licensee failed to notify emergency services though had been requested by the parents.

Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above in 1 out of 1 files, which poses a potential health and safety risk to persons in care. Licensee did not have a current sleep log and past logs did not have child's name, sleep position, or date of log.
Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above in 1 out of 1 staff files which poses a potential health, safety or personal rights risk to persons in care. Assistant did not have an MMR or TB test record on file prior to working with daycare children.
(8) Each family child care home shall have a current roster of children as specified in Health and Safety Code Section 1596.841. This requirement was not met by: Based on record review, 3 out of 9 children present were missing from the facility roster, 11 children were missing physician information, and 5 children were missing from the roster that were on the previous roster given to the LPA on 3/10/26, which poses a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Building or Grounds — Licensee notified the Department about adding an additional living room but she did not wait for Department inspection approval prior to using it.
Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above in as LPA did not observed the fire drill was conducted and documented which poses/posed a potential health, safety or personal rights risk to persons in care.
Personal Rights — LPA learned that on 8/24/2022, licensee admitted to licensing that she needed a second opinion regarding the health status of a daycare child, and she shared private information with another parent who she had consulted for advice.
