Teacher Qualifications and Duties — Based on interview and record review, the licensee did not comply with the section cited above in S3 did not have proof of working towards any units in their file which poses/posed a potential health, safety or personal rights risk to persons in care.
Health-Related Services — Based on observation, the licensee did not comply with the section cited above in 3 emergency medications were missing a prescription label which poses/posed a potential health, safety or personal rights risk to persons in care.
101216.2(e)(1) Teachers Aide Qualifcations and Duties.. This requirment was not met as evidenced by: During review of staff files, LPA observed that staff 3 (S3) and Staff 5(S5) designated as Teachers aids did not have any educational units on file, LPA confirmed that S3 escorts children to restroom and S5 is left alone with children during nap time, where during this visit LPA observed that 2 children were awake.
(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This has not met as evidenced by, facility was unable to provide verification of immunization, which was requested on previous visit 06/25/24.
Buildings and Grounds — Based on observation, the licensee did not comply with this in that the wall behind the toilet in the middle stall of the seahorse classroom had an open hole in the wall, larger than a tennis ball, where plumbing pipes can be reached, which poses/posed a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observation and interview, the licensee did not comply with the section cited above where the rubber surface has a larger than 10in diameter hole, with depth of a childs foot of missing rubber surface which poses/posed a potential health, safety or personal rights risk to persons in care.
Child Care Center Director Qualifications and Duties — Based on observation, interview and record review, the licensee did not comply with the section cited above in by not having a fully qualified director which poses a potential health, safety or personal rights risk to persons in care.
Personnel Records — Based on observation, interview, record review the licensee did not comply with the section cited above in 2/2 assistant teachers do not have the education transcripts in files, which poses/posed a potential health, safety or personal rights risk to persons in care.
