Personal Rights — Based on observation, interviews and record reviews, Licensee did not comply with section cited above, A facility staff/teacher handled children in a rough manner and staff/teacher caused an arm injury to a day care child which posed an immediate health, safety or personal rights risk to persons in care.

Teacher Qualifications and Duties — Based on interviews, observation and records review, Staff 1 did not have required early childhood education to be a qualified teacher, and was left supervising children without a qualified teacher, which poses a potential risk to the health and safety of children in care.

Personal Rights — Based on interviews and observations, a teacher had mishandled a child which caused an injury to the child. This poses an potential risk to the health and safety of the children in care.

Immunizations — Based on observation interview and record review, child C2 did not have completed immunization record, which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, Licensee did not comply with the section cited above. Licensee did not obtain Mandated Reporter Training certification for staff S4 and S12

Personnel Requirements — Based on record review, Licensee did not comply with the section cited above. Licensee did not obtain LIC503Health Screening reports for staff S1 and S3 which posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, Licensee did not comply with the section cited above. Licensee did not obtain immunizations required prior to employment for staff S1 and S4 which posed a potential health, safety or personal rights risk to persons in care.
