Responsibility for Providing Care and Supervision — , Based on obervation (video), and interview, staff did not prevent minor from hitting another minor and dis not ensure proper supervision. This is an immediate risk to health and saftey of children in care.

Teacher-Child Ratio — Based on LPA observations on 05/15/24, 2 teachers were observed supervising 26 children in the Older Twos classroom on 05/22/24 and on 06/05/23, 1 staff supervising 16 children(13 napping, 3 awake including 1 child sitting at the table) in classroom Threes, which posed an immediate risks to the health, safety and personal rights to the children in care.

Responsibility for Providing Care and Supervision — Based on LPA interviews, 7 out of 10 interviewed staff confirmed that S1 was being on their phone 50-100% of the time while S1 was providing care and supervision for the children in the classroom/playground, which posed an immediate risk to the health, safety and personal rights to the children in care.

Teacher-Child Ratio — Based on interviews and records review, teacher S1 and aid S2 were supervising 17 preschool children in the Twos classroom; teacher S4 and aid S3 were supervising 17 preschool children in the Older Twos classroom, which posed an immediatel risks to the health, safety and personal rights to the children in care.

Teacher-Child Ratio — Based on observation, Room 3s has 1 qualified teacher with 13 children. This poses a potential risk to the health, safety and personal rights to the children in care.

Responsibility for Providing Care and Supervision — Based on interviews and declarations from staff 3 (S3) and staff 4 (S4) witnesses on the camera footage of the incident in which Child 2 (C2) exhibited inappropriate behaviors to Child 1 (C1) while at lunchtime.
