Daily Inspection for Illness — Based on interviews four (4) out of four (4) staff members stated that children were allowed to return to school with active lice. This poses an immediate health, safety or personal rights risk to persons in care.

Personal Rights — … Based on interviews, record review and video surveillance the licensee did not ensure the safety of C1 and C2 as S5 grabbed the hair of C1 and S5 placed their hand in front of C2 to stop C2 from running. This poses an immediate risk to the health of the children in care.

Criminal Record Clearance — one (1) staff member did not possess criminal record clearance. This is an immediate risk to the health and safety of children in care.

Observation of the Child — Based on interviews and record review, Child 1 (C1) was not directly observed by Staff 2 (S2) resulting in child running, falling, and injuring their head which causes an immediate risk to health and safety of children in are.

Personal Rights — Based on records review and staff interviews, LPA observed video footage and Unusual Incident Report (UIR) of S14 forcibly laying C2 on cot. 2 (two) staff interviews stated S14 forcibly laid C2 on their cot. This poses as an immediate risk to the health of the children in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above. 2 out of 5 staff did not have proof of immunization against measles. This poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review , the licensee did not comply with the section cited above. 1 out of 5 staff did not have a compelte health screening report. This poses a potential health, safety or personal rights risk to persons in care.

Admission Agreements — Based on record review, the authorized representative of Child 1 (C1) was verbally infromed about incident that occurred on 5/7/25 and was not provided a incident report form to sign which poses a potential risk to children in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation and interview, the licensee did not comply with the section cited above in that three (3) preschool classrooms had several light panels that were not working for several weeks, which poses a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above in that the play yard grass was not kept free of holes, which poses a potential health, safety or personal rights risk to persons in care.
