Criminal Record Clearance — the following, Based on the conducted room observations, interviews and reviewed facility records, the following were observed to be not incompliance. 1. Staff 1, 3, 4, 5, and 6 not associated with the license number 434411985. Staff 1, 3, 4, 5 and 6 supervised in the preschool classrooms. The Director stated that the licensee has the only access to Guardian to process this requirement. Section 101170(e)(2) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility to Request a transfer of a criminal record clearance. This section was discussed with the Director on LPA’s visit dated 8/13/2025. This poses a potential risk to the health and safety, of the children in care.
Child's Records — the following, Based on the conducted room observations, interviews and reviewed facility records, the following were observed to be not incompliance. 2. The Director stated that the LIC 9040 Childcare Roster and LIC 500 Personnel Report, were not up to date and she will update these forms. Section 101221 Child's Records (a) A separate, complete and current record for each child is maintained in the childcare center, and 101217 Personnel Records (a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. These sections were discussed with the Director on LPA’s visit dated 8/13/2025. This poses a potential risk to the health and safety, of the children in care.

Teacher Qualifications and Duties — Based on observation and interview, it was determined that one unqualified staff member was providing care and supervision to daycare children. This poses a potential risk to the health and safety to children in care.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above as the First Aid/CPR training completed by staff is not EMSA approved, which poses a potential health, safety or personal rights risk to persons in care.
