Mandated reporter training — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the staff file review the LPA was unable to review a current Mandated Reporter training certificate for S1, S2 and S4.
Personnel Requirements — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During staff review the LPA was unable to review a valid CPR/1st Aid card for S1 and S4. S1 and S4 are opening staff.
Drinking water lead testing — Based on observation, interview and record review, the licensee did not comply with the section cited above in the facility uses tap water for food preparation and to water vegetable garden, offered to clients for consumption; and did not retest water outlets, after receiving a lead test report on 12/10/2019 documented results of a lead exceedance on an outlet in a lunch preparation area of 42.0 ppb., which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review of S2, S2 and S3 it was discovered the Mandated Reporter certificates were not current or not available for review.
Staff immunization requirements — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review of S3 it was discovered there were no immunizations available for review including a TB clearance.
Personnel Requirements — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review of S3 it was discovered there was not an LIC 503 Health Screening available for review.
Health-Related Services — Based on the observation, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During facility tour LPA observed expired medication for children who are still attending. The medication was not administered to the children.
