Upon arrival, LPA Farida observed one teacher with 13 preschool children on 10/24/2023. Teacher/Facility Representative stated that one teacher was on break.

Buildings and Grounds — Based on tour observation indoor and outdoor of the toddler & preschool, LPA toured outdoor play areas and observed children's bikes, cars, waffle blocks, play house, chairs, easel, and broken toys, one play structure was rusted in several places, a part of the play ground structure was removed leaving behide a piece of concrete with screws sticking out, and uneven walking surfaces tripping hazard to children in care (toddler & preschool outdoor area). LPA observed disrepair items were not placed inaccessible to children in care. LPA toured the preschool classroom and observed broken blinds, dusty ceilings & indoor play area stairs dusty, sprider webs, rip carpet, dirty rug, stain walls & chip paint, a lose baseboard in children bathroom. LPA observed rip napping mats, rip carpet, stain walls & chip paint in the toddler classroom. This poses a potential risk to the health and safety of children in care.
Drinking water lead testing — Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Facility Representative stated that Water Lead Testing was not completed for this facility during inspection on 10/24/2023.
Staff immunization requirements — Based on record review and interview from LPA Farida on 10/24/2023 the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA Farida did not observe immunization records for MMR, Tdap and Influenza for staff S2 and S3.
Mandated reporter training — Based on record review and interview from LPA Farida on 10/24/2023, the licensee did not comply with the section cited above for one staff which poses a potential health, safety or personal rights risk to persons in care.
Drinking water lead testing — Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. FAcility Representative stated that Water Lead Testing was not completed for this facility.
Disaster and Mass Casualty Plan — Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Facility Representative was unable to find the fire drill log during today's inspection.
