Staff-Infant Ratio — On May 29, 2024 LPA witness the facility being out of ratio in the infant classroom. LPA observed two (2) aids (S1 & S2) supervising six (6) infants. This poses an immediate risk to health and safety of the children in care.
Criminal Record Clearance — Based on observation, interview, and record review, staff (S-1) who was present at the facility has cleared fingeprints, but fingerprints are not associated to facility. This poses a potential health and safety risk to children in care.
Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on observation and interview, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPA observed two baby bouncers in the infant nap area during today's visit.
Responsibility for Providing Care and Supervision for Infants — Based on interview and record review, the licensee did not comply with the section cited above for all infants which poses an immediate health, safety or personal rights risk to persons in care.
Staff-Infant Ratio — Based on observation and record review, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. Through file review, LPA observed that the teacher in the infant classroom did not have 3 units in Infant and toddler care which places the facility out of ratio since there is no fully qualified teacher in the classroom.

Fixtures, Furniture, Equipment and Supplies — On April 30, 2024 LPA did a physical plant inspection and observed seven (7) out of the eight (8) infant mattress were rip and not maintained in good condition. Which poses/posed a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on tour observation indoor and outdoor in the Infant Center, LPA observed broken toys and rusted swings outdoors that were not placed inaccessible to children in care. LPA also observed uneven walking surfaces and tree roots making it a tripping hazard to children in care. LPA also observed an old wood shade structure that is a health and safety concern. LPA observed rip mattresses in all infant cribs, two broken cribs, broken blinds, unsanitary carpet in crib area and in infant indoor playground structure, stains in walls & chip wall paint. This poses a potential risk to the health and safety of children in care.
Drinking water lead testing — Based on interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Facility has not completed Water Lead Tessting.
Disaster and Mass Casualty Plan — Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Facility Representative was unable to find the fire drill log.
Infant Care Teacher Qualifications and Duties — Based on observation, interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Health-Related Services — Based on observation, interview and record review, the licensee did not comply with the section cited above for one child in care (C3) which poses a potential health, safety or personal rights risk to persons in care.
