Staff Silvia Tovar and Abigail Arechiga are missing proof of current Mandated Reporter Training certificate on file. Limitations on Capacity and Ambulatory Status — Based on observation, the licensee did not comply with the section cited above. LPA observed site director's 7 month old child on site (in the office). Also observed was a baby walker. Ms. Arechiga stated that her child stays in the office with her and that the walker is also for her child's use. This deficiency poses a potential health, safety or personal rights risk to persons in care.
