Health-Related Services — Based on observation, interview, and record review, the licensee did not comply with the section cited above as Child #2's Albuterol expired on 11/2024, which poses a potential health, safety or personal rights risk to persons in care.
Food Service — LPA observed blood worm cubes in the blister packs used for animals stored on the same shelf with vanilla ice cream and on the top of the frozen waffles without original box .LPA observed containers with worms with the holes on the lid in the plastic bag stored next to the vegetables, milk, and lemon juice. LPA observed open container without a lid. LPA observed paper cups covered with plastic wrap.
Food Service — Facility did not have a dishwasher and did not sanitize dishes using alternative comparable method. Per staff members, dispenser for sanitizing was not working and dishes were not sanitized.
