LPA observed Paula A. Guevara Grajales is cleared but was not associated to the facility which poses/posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — 09/28/23 incident was submitted to the San Jose Regional Office on October 04, 2023. This poses an immediate risk to the health and safety of the children in care.
