Teacher-Child Ratio — Based on observation, the licensee did not comply with the section cited above, LPA observed 15 children with 1 staff in the Great Room which poses an immediate health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on interviews it was disclosed that S2 and S5 have been alone with more than 12 children at one time. This poses an immediate health and safety risk to children in care.

Criminal background check — Based on inyterview and record review, the licensee did not comply with the section cited above in the lead teacher, did not have the proper live scan clearance granted prior to working at facility on 8/12/2024, which poses an immediate health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — Based on record review, the licensee did not comply with the section cited above in the facility has not conducted a disaster drill since 05/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above in LPA observed a wooden bench where the sides are loose and the paint in chipped. LPA also observed bikes without rubber handles which poses/posed a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above LPA observed trash, debris and spiderwebs on outdoor yard and equipement which poses/posed a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on observation and interview the licensee did not comply with the section cited above LPA observed children sitting in the hallway near the restroom alone. LPA spoke with S1 who stated when there is no assistance, children are left unsupervised while they assist the children in the restroom which poses/posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in LPA observed S2 and S3 did not have a current mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on observation, the licensee did not comply with the section cited above in LPA observed S2 and S3 did not have immunization records in their files which poses/posed a potential health, safety or personal rights risk to persons in care.

Child Care Center Director Qualifications and Duties — Based on observation and record review the licensee did not comply with the section cited above in director has not been present at the facility in the past three years when the department is conducting and inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review the licensee did not comply with the section cited above S2 does not have a current health screening which poses/posed a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on interviews and observation the facility did not have hot water during the first inspection 11/03/2025. Per S2, they utilize hot water to wash the children's utensils. This poses a potential health and safety risk to children in care.

Disaster and Mass Casualty Plan — Based on interview and record review, the licensee did not comply with the section cited above in disaster drill has not been conducted since one year ago, which poses/posed a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above in play equipment and climbing ladders are arranged and spaced creating conflict of activities not meeting fall zone requirements, which poses/posed a potential health, safety or personal rights risk to persons in care.
