LPA observed gaps between the foam tiles that can cause a tripping hazard for infants during outdoor play which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 3 of 3 staff are missing influenza, 2 of 3 staff are missing measles, 1 of 3 staff is missing pertussis which pose a potential health, safety or personal rights risk to persons in care.
