Based on interviews conducted and admission made indicating that Child #2, Child #3, Child #4, Child #5 and Child #6 were made to stand in a bent down position touching their toes to correct their behaviors while S1 was restraining Child#1 which poses an immediate health, safety, and personal rights risk to children in care.
TrustLine provider — registry record not kept current — Licensee failed to safeguard C1’s personal rights by allowing & encouraging S3 to spank C1 in the facility. Interview of S3 revealed that they are an employee of the facility and a relative of C1 and having been misled by S1 about C1’s misbehavior, they physically punished C1. This poses an immediate health, safety, and personal rights risk to children in care.
Personnel Requirements — Licensee allowing physical abuse of 6 children, based on interviews conducted with S1, S2 & S3 who have failed to safeguard C1 from corporal punishment when S3 physically punished C1 as a form of discipline, and S1 restrained C1, which poses an immediate health, safety, and personal rights risk to children in care.
Buildings and Grounds — During a walk through of the physical plant, LPAs observed adult scissors in each classroom, accessible to children. At 9:08AM, LPAs observed a shed/storage unit in the outside play yard containing hazardous cleaning compounds (such as Comet, Lysol, Bleach, Rubbing Alcohol and Floor Polish) unlocked and accessible to children. At 9:23AM, LPAs observed knives in the kitchen cabinet and cleaning supplies underneath the kitchen sink left unlocked and accessible to children. This poses an immediate health, safety or personal rights risk to persons in care.

Criminal Record Clearance — Based on record review, the licensee did not ensure Kyra Bacon (date of hire 12/4/23) was not associated to the facility which poses a potenial health, safety, or personal rights risk to persons in care. A civil penalty of $500 has been assessed.
Child Care Center Director Qualifications and Duties — Licensee Representative/Director is missing 3 Units Child Family and Community course. This poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — During today’s visit LPA reviewed staff files and observed that Licensee Representative/Director is missing proof of measles and Staff 1 is missing proof of influenza, pertussis and measles. This poses a potential health, safety or personal rights risk to persons in care.
