Disaster and Mass Casualty Plan — Based on interview and record review, the licensee did not comply with the section cited above. Facility has not conducted a fire drill since May 2024. This deficiency poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on interviews conducted with staff, it was determined that a staff was rough with a child when she grabbed her by the hand and roughly sat her down on a chair.
Criminal Record Clearance — Based on LPA's review of the fingeprint association database, 3 staff were not associated with the infant license. These staff were associated to the preschool license.
Reporting Requirements — Site Director failed to report an incident involving an Aide who was left to supervise 6 children in the classroom. Fully qualified teacher had to use the restroom and was unable to find coverage.
Staff-Infant Ratio — An aide was left to supervised 6 infants (this happened sometime in November). Staff did not remember the exact date.
