The requirement was not met as evidenced by, Based on 2 of 4 children identified as potential witnesses stated that Staff #1 (S1) told a child that they would place tape over their mouth if they didn't stop talking and 3 of 5 staff members identified as potential witnesses stated they have witnessed or overheard S1 talk to the children in an aggressive tone and make inappropriate comments to children. This is a potential health and safety risk to children in care.
Personal Rights — Based on record review, and interviews, the facility did not comply with the section cited above in that, (S1) was observed pulling (C1)'s arm, in a rough manner, which poses a potential health, safety or personal rights risk to children in care.
