Buildings and Grounds — THIS IS AN AMENDED REPORT DELIVERED ON DECEMBER 3, 2025 Based on observation, the licensee did not comply with the section cited above because on the playground has insufficient cushioning with several holes causing tripping hazard for the children in care which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on records review 2 of 6 staff files were missing immunizations required which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on records review 2 of 6 staff files were missing staff Health Screening which poses/posed a potential health, safety or personal rights risk to persons in care.
Teacher Qualifications and Duties — Based on records review, staff S4 and S5 in Room 1 with 8 infants did not have proof of 12 ECU units in their file leaving the classroom without a fully qualified lead teacher which poses/posed a potential health, safety or personal rights risk to persons in care.
Indoor Activity Space for Infants — Based on interviews conducted by LPA, infant/toddlers are allowed to use the preschool activity space with the preschoolers 30 days prior to their second birthday. This poses a potential health, safety or personal rights risk to the children in care.
