Responsibility for Providing Care and Supervision — Based on the facility's self reported UIR, S2-S3 interviews, S2, S3 & S5 written statements and documents received a child was left without supervision in the facility hallway during a transition from one preschool classroom to another for approximately 1-5 minutes. This poses an immediate health and safety risk to children in care.

Civil penalty in addition to suspension or revocation — On 10/25/2024, C1 left the premises for roughly 1-2 minutes resulting in an absence of supervision, which poses an immediate Health and Safety risk to children in care.

To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This regulation was not met as evidenced by Based on record review and interview, the facility did not request guidance from Individual Education Services for child 13's behavior until 5/15/26 which resulted in multiple incidents of C13 injuring other children without an established behavior plan to improve or manage the behavior.

Plan of Operation — Based on staff interview and review of facility policy, the facility is required to provide an Incident/Accident Report by pick up to the parents of any child who was injured. On 4/3/26, an Incident/Accident report was not given to the parent of a child at the end of the day of the incident which poses a potential risk to the health, safety, or personal rights of children in care.

Personal Rights — Based on AD, S2, S8-S9 interviews, 2 staff handled 2 different children in a rough manner which poses a potential health, safety or personal rights risk to persons in care.

Limitations on Capacity and Ambulatory Status — Based on CD and S4 interviews that stated care and supervision is being provided for school age children in the preschool classroom at morning drop off until being bused to school which poses a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on LPA observations of cots being too close together, AD & S9 interview, S1 sets cots out while children are outside playing before lunch and S3 & S4 stated they set cots up before children go outside which poses a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on interviews and observations from CD & LPAs, feces was not cleaned or sanatized from a toilet seat from 12/3/25 to 12/8/25, which poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on interviews from CD, AD & Staff, an incident occurred when a rodent fell from a ceiling tile after being caught in a trap with no daycare children present, which poses a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on an interviews, Direcor did not notify and submit a UIR to CCL. This posed a potential health, safety and/or personal rights risk to children in care.

Responsibility for Providing Care and Supervision — Based on interviews, and observations, evidence shows that a child was in the restroom alone and without staff supervision. This poses a potential health, safety and/or personal rights risk to the children in care.
