Infant Care Activities — Based on observation and record review, the licensee did not comply with the section cited above in 2 out of 5 children (C3-C4) LPA observed C3 and C4 asleep on their stomach and Provider did not complete Individual Infant Sleep Plan LIC9227 Section D that stated the infants were observed by staff to be capable of rolling from their back to their stomach and their stomach to their back which poses an immediate health, safety or personal rights risk to persons in care.

Indoor Activity Space for Infants — Based on LPA observed 3 two year old children being supervised in the infant (birth to 24 months) classroom today.
Disaster and Mass Casualty Plan — Based on observation and record review, the licensee did not comply with the section cited above as DL did not provide documention of disaster drills conducted in the last six months which poses/posed a potential health, safety or personal rights risk to persons in care.
