Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in that C3 did not have a completed and signed physician's report. The medical assessment report was missing results of a test for tuberculosis and record of any infectious or contagious diseases which poses a potential health, safety or personal rights risk to persons in care.
