LPA observed that staff 1 and staff 2 did not have a health screening form (LIC 503) in their file including a test for tuberculosis. This poses a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on staff and parent interviews. Six staff members stated that the facility does not have an isolation area. Two parents stated that they pick up their sick children in the classroom. This poses a potential health risk to persons in care.

Mandated reporter training — Based on record reiew , the licensee did not comply with the section cited above in two out of seen staff did not have a valid mandated reporter completion certificate which poses a potential health, safety or personal rights risk to persons in care.
Infant Needs and Services Plan — Based on interview, Director did not comply with the section cited above in 2out of 4 infants did not which poses/posed a potential health, safety or personal rights risk to persons in care.
