Teacher-Child Ratio — Based on record reviews and interviews, the licensee did not comply with the section cited above in that there were at least 3 out of 3 randomly selected dates that showed multiple rooms where the facility was operating outside of the 1:12 teacher to preschool age child ratio for 5 minutes or more.
Responsibility for Providing Care and Supervision — Based on evidence gathered through interviews, documentation obtained & the agency's self-reported notification that a child was left unsupervised on 10/31/2024 for 2-3 minutes, This is an immediate risk to the health and safety of children in care. A civil penalty was issued for $1,000.00

Responsibility for Providing Care and Supervision — Based on interviews conducted and documentation obtained, 2 children were found in the classroom unattended. This is an immediate risk to the health and safety of children in care.
Personal Rights — Based on interviews conducted and records review it was found that C2 was put on a behavioral plan after approximately year of C2 violating children's personal rights. Facility failed to provide a safe environment for the children in care. This poses an immediate health, safety or personal rights risk to persons in care.
Criminal background check — Based on a record review , the licensee did not comply with the section cited above in 1 out of 10 staff wasn't associated to the facility, which poses an immediate health, safety or personal rights risk to persons in care.

Notice to parents that site visit reports are available — Based on record review, the licensee did not comply with the section cited above in that 3/5 children's records reviewed (Child 3 [C3], Child 4 [C4], Child 5 [C5]) did not contain the verification of receipt of licensing reports (LIC9224) for the Type A citations issued on the dates of: 7/10/24, 8/29/24, 9/17/24 (2 reports on this date), and 11/14/25. These children did not need these documents completed while they were enrolled in the infant program, however they did need them completed after transitioning to the preschool program during the year of 2025. This poses/posed a potential health, safety or personal rights risk to persons in care.
