Personnel Requirements — Based on observations and interviews, the licensee did not comply with the section cited above as there was a ratio of one teacher to fourteen children during a water activity, which poses an immediate health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observations and interviews, the licensee did not comply with the section cited above as there was a large infatable water slide that contained approximately 2 feet of water. Interviews disclosed that the water slide was set up in th morning and remained accessible throughout the day while containing standing water even when children were done using the water slide, which poses an immediate health, safety or personal rights risk to persons in care.

The licensee shall ensure that each child is accorded the following personal rights:To be accorded dignity in his/her personal relationships with staff and other persons... — requirement not met

Staffing for Water Activities — Based on records review and interviews, the licensee did not comply with the section cited above as the facility did not have an adult in site who had a valid water-safety certification while children partook in water activities, which poses a potential health, safety or personal rights risk to persons in care.

Limitations on Capacity and Ambulatory Status — Based on observations and interviews, the licensee did not comply with the section cited above as the school age classroom was observed utilizing the side yard (southern end) of the facility that was not approved for child care use, which poses/posed a potential health, safety or personal rights risk to persons in care.

Drinking water lead testing — Based on records review, observations, and interviews, the licensee did not comply with the section cited above as the kitchen faucet was not tested for lead contamination and was routinely used for food preparation, cooking, drinking water, and washing cookware and utensils, which poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observations, the licensee did not comply with the section cited above as the cabinet inside the Jr's classroom was not locked and was left open. Upon inspection LPAs observed cleaning solutions readily available to chidlren in care, which poses/posed a potential health, safety or personal rights risk to persons in care.
