Responsibility for Providing Care and Supervision for Infants — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the facility tour the staff acknowledged C4 can roll from their stomach to back on their own. During children’s file review the LPA was unable to review an LIC 9227 with Section C completed.
Modifications to Infant Needs and Services Plan — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review the LPA was unable to review current Infant Needs & Service Plans for C1 and C3.
Immunizations — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During children’s file review the LPA was unable to review current immunizations for C3.
Infant Care Teacher Qualifications and Duties — Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that of the 6 staff present in the two infant classrooms, only 1 staff was determined to be a qualified teacher, while the other staff have certificates on file whose accreditation could not be validated at this time which poses a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision for Infants — Based on record review, the licensee did not comply with the section cited above in that LPAs observed 15 minute sleep checks were incomplete and did not have initials of person who conducted the sleep checks which poses a potential health, safety or personal rights risk to persons in care.
