Teacher-Child Ratio — Based on interview and documentation, there was an unqualified aide assistaningt a teacher with 16 during outdoor water play where a child was injured

Subsequent to initial licensure, a person specified in subdivision (b) who is not exempt from fingerprinting shall obtain either a criminal record clearance or an exemption from disqualification, pursuant to subdivision(f) of this section or Section 1522.7, from the State Department of Social Services prior to employment, residence, or initial presence in the facility. Personal Rights — Based on LPAs interviews and records review staff member has admitted to tugging a daycare child's hair which is a violation of the Title 22 Regualtion.

Teacher Qualifications and Duties — Based on record review the licensee did not comply with the section cited above in an unqualified teacher was alone with 12 children which poses an immediate health and safety risk to persons in care.

Teacher Aide Qualifications and Duties — Based on staff interview staff could not provide documentation supporting whether the aide had qualifying educational units to aide a fully qualified staff.

Isolation for Illness — Based on LPA interviews staff stated they do not utalize the designated isolation area when children are ill which posed a potential health, safety, or personal rights risk to the children in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in that they did not keep a LIC 503 -Health Screening form for a staff member list on the LIC 859 Staff file review forms which posed a potential health, safety or personal rights risk to persons in care.

Drinking water lead testing — Based on interview, record review, the licensee did not comply with the section cited above in that they had not tested for lead exceedance as of the date of inspection 11/1/23 which posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in several staff did not have a mandated reporter certificate for the department to review which posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in that a facility staff did not have any immunizations in their file for the department to review which posed a potential health, safety or personal rights risk to persons in care.
