Responsibility for Providing Care and Supervision for Infants — Based on LPA record review, the licensee did not comply with the section cited above, C1: sleep log for 10/31/2023 documented the last 15-minute check was at 1:25 p.m., C1 nap time duration was documented 12:25 p.m. - 2:08 p.m., in which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based LPA record review, the licensee did not comply with the section cited above, S2 is missing proof of Influenza vaccination or statement declining Influenza, S3 is missing proof of MMR, TDAP and Influenza vaccination or statement declining Influenza vaccination in which poses/posed a potential health, safety or personal rights risk to persons in care.
