Activities/Napping — Based on LPA observation, there were 16 children in the Preschool A room supervised by only one teacher and 20 children in the TK room supervised by only one teacher. It was nap time but not all children were napping which poses a potential health, safety, or personal rights risk to children in care.

Responsibility for Providing Care and Supervision — Based on interviews conducted with AD, S4, S6, S7 & P1, adequate supervision was not provided to children in care, resulting in a C2 eating a crayon which poses a potential health, safety and/or personal rights risk(s) to children in care.

Isolation for Illness — Based on interviews with staff and parents, child 7 was suspected of having Hand, Foot, and Mouth Disease on 7/16/25 and was moved to a classroom belonging to the infant program instead of the center's isolation area which poses a potential health, safety, or personal rights risk to persons in care.

(a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. The following was not met as evidenced by: Based on interviews with staff and parents, child 7 was suspected of having Hand, Foot, and Mouth Disease on 7/16/25 and was moved to a classroom belonging to the infant program with two children from the infant program present instead of the center's isolation area which poses a potential health, safety, or personal rights risk to persons in care.

Limitations on Capacity and Ambulatory Status — Based on Staff and Parent interviews and facility Child Supervision Record obtained by LPA 2 one-year old infants were being supervised in the Discovery Preschool Two's room over several days, which poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on LPA's observation the facility did not comply with the section cited above. The diaper on the bathroom floor, rubber mats and cartet floor mats with exessive dirt and soap stained cardboard box, which poses a potential health, safety, or personal rights risk to persons in care.

(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. The following has not been met as evidenced by: Based on observation and record review the a classroom in the facility had only one teacher, staff 7 (S7) supervising 13 children which poses a potential health and safety risk to the children in care.

Mandated reporter training — Based on record review, staff 3, staff 4, and staff 5 (S3, S4, and S5) did not possess current mandated reporter training certificates which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, staff 3 (S3) did not possess proof of immunization against measles which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the facility did not possess record of staff 3 and staff 4's (S3 and S4) LIC503 Health Screening which poses a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the facility did not possess record of child 1, 2, and 3's (C1, C2, and C3) physician's report which poses a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on record review, the the facility did not possess record of child 2's (C2) immunizations which poses a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on record review, child 1's (C1) identification and emergency information card was not signed and child 2's (C2) personal rights and parent's rights forms were not signed which poses a potential health, safety or personal rights risk to persons in care.
