Staff-Infant Ratio — Based on statement provided by AD, 7 staff and LPA’s observations of 2 staff supervising 9 children at 7:00 am, 2 staff supervising 11 children at 7:35 am and 4 staff providing care and supervision 21 children at 8:14 am on 12/12/24.

Reporting Requirements — Based on CD UIR notification received on 10/27/25 and document LPA received on 1/12/26 and CD admitting to not reporting the outbreak in a timely manner and the Department did not received the UIR notification within the Department’s next working day during normal business hours. This poses a potential health, safety or personal rights risk to persons in care.
Isolation for Illness — Based on interviews with staff, child 5 was suspected of having Hand, Foot, and Mouth Disease on 7/7/25 and was not moved to the centers isolation area which poses a potential health, safety, or personal rights risk to persons in care.
Reporting Requirements — Based on AD admitting to not reporting the outbreak that began on 07/07/2025 and the Department has no record of the incident reported to the Department within the Department’s next working day during normal business hours. This poses a potential health, safety or personal rights risk to persons in care.
Infant Care Activities — Based on AD interview and 8 staff interviews the holding room is used as activity space when facility is over capaity,which poses a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space for Infants — Based on observation, at 9:35 AM, children from the infant license were co-mingling with children from the pre-school license in the playground outside which poses a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision for Infants — Based on observation and record review, at 9:40 AM, LPA observed child 1 (C1) and child 2 (C2) sleeping on their stomachs and C1 and C2's LIC9227 Individual Infant Sleeping Plans did not indicate that they are able to roll over which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, staff 2, 3, and 6 (S2, S3, S6) did not possess current Mandated Reporter Training Certificates which poses a potential health, safety or personal rights risk to persons in care.
Health-Related Services — Based on observation, the inhaler for child 5 (C5) was expired which poses a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on record review, child 2 (C2) and child 4 (C4) did not possess record of physician's reports which poses a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, child 2 (C2), child 4 (C4), and child 5 (C5) did not possess record of complete immunizations which poses a potential health, safety or personal rights risk to persons in care.
