The licensee shall ensure that each child is accorded the following personal rights: To be accorded dignity in his/her personal relationships with staff and other persons. This was not met as evidenced by: During interviews w pertinent parties, it was admitted that the staff in question made an inappropriate statement in a child’s ear. This is a risk to the health/safety to children in care.

Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. (1)Storage areas for poisons shall be locked. Based on LPAs observation, there were two disinfecting sprays which both had: "Medical Emergency" phone numbers to contact. Additionally, the following note was on them "CAUTION: Causes moderate eye irritation. Avoid contact with eyes or clothing. Wash thoroughly with soap and water after handling". This was not kept locked and was accessible to the school age children.

Outdoor Activity Space — Based on observation the Licensee did not meet the above regulation which posed/poses a potential health and safety risk to persons in care. LPAs observed broken glass and a cigarette butt in the outdoor activity area.

(a) In addition to the sign-in procedure requirement of Section 101226.1(b), the licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the center that shall, at a minimum, include the following:(1) The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. Based on director interview &record review, facility did not have sign in/out sheets available for authorized representatives on 11/01/24, resulting in no signatures for sign in. This poses a potential

(a) Every child care center shall have all admission policies in writing and available to the public. The policies shall coincide with the limitations stated on the license, and shall include, but not be limited to, the following:(6) Transportation arrangements, if any. Based on interviews with pertinent parties, a child was left at their elementary school and forgotten. The elementary school had to call the facilty and the chidl was picked up approximately 45 minutes-1 hour later. This poses/posed a potential health and safety risk to person in care.

(a) A separate, complete & current record for each child is maintained in the CCC. During record review, 2 of 4 children's files were unavailable and 1 of 4 children's files were incomplete. This poses a potential health & safety risk to the children in care. — requirement not met

Personnel records shall be maintained on the licensee, administrator, and each employee, and shall contain specified information. During record review, 2 of 2 staff files were incomplete. This poses a health& safety risk to children in care. — requirement not met

Drinking water lead testing — Based on record review and directors own admission, the facility did not comply with the section cited above. Centers should have had water tested for lead contamination levels no later than January 1, 2023. This center was not able to get their lead testing done. This poses/posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the facility did not comply with the section cited above in 1 out of 2 persons. A staff did not have an updated mandated reporter training (MRT) for "Child Care Providers", as required. This poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the facility did not comply with the section cited above in 1 out of 2 persons. One staff did not have a health screening on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
