Responsibility for Providing Care and Supervision — Based on the self-reported incident and staff interviews conducted and record review, it was determined that C1 was left in the classroom alone without supervision, which poses an immediate risk to the health and safety of the children in care.

Teacher-Child Ratio — Based on the self-reported incident from interviews conducted and record reviews, it was determined that S1 was supervising 13 children alone. Which poses an immediate risk to the health and safety of the children in care.

Outdoor Activity Space — Based on observation and interview the licensee did not comply with the section cited above. LPA observed climbing equipment to have a slippery climbing surfaces and thinning areas of the rubber cushioning which is not enough to absorb falls. Staff stated there are incidents when children would slip from the structure and land on their faces. This poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 3 staff were missing Health Screening and TB Test which poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observations and interviews conducted, it is determined that there were pest droppings in the facility. This poses a potential risk to health and safety of children in care.

Written directives for drinking water lead testing — Based on facility Lead sampling tests it was discovered that outlet P,Q,S,N,K had high levels of lead. This poses a potential risk to the health of children in care.
