Outdoor Activity Space — Based on observation and interview the licensee did not comply with the section cited above. LPA observed climbing equipment to have a slippery climbing surfaces and thinning areas of the rubber cushioning which is not enough to absorb falls. Staff stated there are incidents when children would slip from the structure and land on their faces. This poses a potential health, safety or personal rights risk to persons in care.
