Staff-Infant Ratio (b) There shall be a ratio of one teacher for every four infants in attendance. This was not met as evidenced by: Based on observation and interview, on 05/29/2026, a confirmed incident occurred resulting in two teachers aids supervising 9 infants without direct supervision from a qualified teacher. This is an immediate health safety and personal rights risk to children in care.

Staff-Infant Ratio — Based on observation and interview, the licensee did not comply with the section cited above. LPA observed one staff member supervising 5 infants. The facility was unable to immediately resolve the infant/teacher ratio due to staffing limitations around 9:00am. Additional staffing arrived around 10:00am, restoring the ratios. This poses an immediate health, safety or personal rights risk to persons in care.

Infant Care Teacher Qualifications and Duties (f) Teachers shall visually observe aides whenever aides are working with infants… This was not met as evidenced by: Based on observation and interview, on 05/29/2026, a confirrmed incident occurred resuting in two teachers aids supervising 9 infants without direct supervision from a qualified teacher. This poses a potential health safety and personal rights risk to children in care.

Reporting Requirements (d) Upon the occurrence, ... a report shall be made to the Department by telephone or fax within the Department's next working day... In addition, a written report containing the information... shall be submitted to the Department... This was not met as evidenced by: Based on observation and interview, on 05/29/2026, a confirmed incident occurred resulting in lack of appropriate supervision and staff/child ratios. This was not reported to licensing when it occurred. This poses a potential health safety and personal rights risk to children in care.

Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (2)To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This was not met as evidenced by: Based on observation and interview, on 06/10/2026 LPA observed and recorded information that napping cots were provided and utilized in the toddler room for infants under the age of 1 year. This poses a potential health, safety or personal rights risk to children in care.

Infant Care Center Sleeping Equipment (b) A crib or portable-crib… shall be provided for each infant who is unable to climb out of a crib. This was not met as evidenced by: Based on observation and interview, on 06/10/2026 LPA observed and recorded information that napping cots were provided and utilized in the toddler room for infants under the age of 1 year. This poses a potential health, safety or personal rights risk to children in care.

Waivers and Exceptions (b) The Department has the authority to approve the use of alternate concepts, programs, services… (1) Such alternatives shall be carried out with provisions for safe and adequate services… This was not met as evidenced by: Based on observation and interview, on 06/10/2026, the toddler room was not operating by approved waiver guidelines by allowing infants under age 1 to nap on cots in the toddler room's activity space. This poses an potential health safety and personal rights risk to children in care.

Staff immunization requirements — Based on LPA’s observation and Staff records reviewed, the licensee did not comply with the section cited above, Staff 4 did not have proof of MMR and Tdap vaccine on file, which poses a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on LPA’s observation and Infant records reviewed, the licensee did not comply with the section cited above, Child 3 did not have proof of immunizations on file, which poses a potential health, safety or personal rights risk to persons in care.
