Responsibility for Providing Care and Supervision (a)The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time,...... Supervision shall include visual observation. The above requirement was not met as evidenced by: a self reported incident interviewing 3 staff, and admitting of S1 leaving C1 unattended in transitioning from the playground to restroom for 2 minutes. This is an immediate risk to the health and safety of children in care.

(a)The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. The requirement was not met as evidenced by interviewing 15 staff of whom 4 staff, and one parent confirmed witnessing staff # 1 yells at children, the other staff confirmed Staff # 1 screams across the classroom “stay in your bed.” The voice of staff # 1 is stressed out.” Staff # 1 created tense to the environment “Staff # 1’s voice is loud.” Staff # 1’s voice is in a disciplinary way.” “The voice of staff # 1 shows frustration”. This is an immediate risk to the health and safety of children in care.

Teacher-Child Ratio — Based on a complaint report, interviewing staff, admission of S1, S2, S3 and reviewing Child Supervision Record (CSR) that S1 was left alone with 13 preschool children for 10 minutes on 4/21/2026 until S2 arrived. This is a potential risk to the health and safety of children in care. An immediate civil penalty of 250.00 is hereby assessed due to a repeat violation of the same on previous dates of 2/3/2026 and 2/17/2026.

Discipline — Based on self reported incident, interviewing staff, children, and by witnessing the incident by S2, and admission of S1 that child's shirt was pulled from the back to remove the child from playing with water was found out to be inappropriate. This is a potential risk to the health and safety of children in care.

Personal Rights — Based on record review and interviews, staff did not ensure day care child was provided undergarments which poses a potential risk to the health and safety and personal rights of the children in care.

Teacher-Child Ratio — Based on a complaint report, interviewing staff, admission of S1, S2, and reviewing Child Supervision Record (CSR) that S1 was left alone with 13 preschool children for 10 minutes on 2/13/2026 until S2 arrived. This is a potential risk to the health and safety of children in care. An immediate civil penalty of 250.00 is hereby assessed due to a repeat violation of the same on previous date of 2/3/2026.

Teacher-Child Ratio — Based on self reported incident, interviewing staff, reviewing Child Supervision Record (CSR) that S1 was left alone with 14 and 15 children for 4 to 5 minutes and S2 with 16 children for 2 minutes on 1/30/2026 in the morning. This is a potential risk to the health and safety of children in care. An immediate civil penalty of 250.00 is hereby assessed due to a repeat violation of previous date of 11/18/2025.

Teacher-Child Ratio — Based on self reported incident, interviewing 3 staff, reviewing Child Supervision Record (CSR) that S1 left S2 with 14 preschool children on the playground between 5 to 10 minutes while being on 10 minute break on 11/6/2025. This is a potential risk to the health and safety of children in care.

101226.2(a)- Isolation for Illness A center shall be equipped to isolate and care for any child who becomes ill during the day. The above requirement was not met as evidenced by interviewing staff during a complaint investigation that S3, S4, S6, S7, and S8 disclosed that sick children are not isolated from other children. This is a potential risk to the health and safety of children in care.

101212(d) Reporting Requirement- Upon the occurrence, during the operation of the childcare center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working within seven days following the occurrence during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department . The requirement was not met as evidenced by interviewing staff confirming they reported staff’s yelling. The facility failed to report it to the Department as it was required to. This is a potential risk to the health and safety of children in care.
