Responsibility for Providing Care and Supervision for Infants — Based on interview and record review, staff failed to prevent an infant from falling out of his feeding chair and hitting his nose on the ground, which poses an immediate health and safety risk to children in care.

Responsibility for Providing Care and Supervision for Infants — Based on interview and record review, a child was alone in the bathroom for up to 4 minutes without staff's knowledge, which is an immediate health and safety risk to child in care.

Responsibility for Providing Care and Supervision for Infants — A child was left outside on the playground without supervision which poses an immediate risk to the health, safety and personal rights of the children in care.

Personal Rights — staff member allowed an inafnt to crawl into a cabinet and then locked the child inside. This poses an immediate risk to the health and safety of the children in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above staff Yolanda Gomez and Debra Lynn does not have current mandated reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff-Infant Ratio — Based on observation, the licensee did not comply with the section cited above, as teacher (6 units) is with 8 infant children with aide, which poses/posed a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on interview, the licensee did not comply with the section cited above in, director did not submit director's packet or notify CCLD. Diane Tapia hired as director in February 2025, which poses/posed a potential health, safety or personal rights risk to persons in care.

Food Service — Based on observation, the licensee did not comply with the section cited above as there were mulitple expired Gerber baby food boxes in the infant room, which poses/posed a potential health, safety or personal rights risk to persons in care.

Immunizations — All five (5) children's files reviewed showed out of date immunization records. Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Sign In and Sign Out — Eight (8) children were not signed in by their authorized representative. Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision for Infants — A child sustained an injury in care due to lack of supervision which poses a potential risk to the health and safety of the children in care.

Infant Care Food Service — One (1) infant in care was accidently given expired formula while in care. This poses a potential risk to the health, safety and personal rights of the children in care.
