Criminal background check — Based on record review, the licensee did not comply with the section cited above as it was found that four staff members are not associated with preschool license, which poses an immediate health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on observation and record review, the licensee did not comply with the section cited above as it was found that there was one teacher and one aid with 21 children in "Discovery" classroom, which poses an immediate health, safety or personal rights risk to persons in care.

No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement is not met as evidenced through interviews, it was found that two children were left unsupervised at the facility. This poses an immediate health and safety risk to children in care.

Criminal background check — Based on record review, the licensee did not comply with the section cited above as the facility has two employee that does not have the clearance on record and three employees that has clearance but not associated with this facility, which poses an immediate health, safety or personal rights risk to persons in care.

The licensee shall ensure that each child is accorded the following personal rights: To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement is not met as evidenced through staff interviews, it was found that a child sustain injuries while in care over the period of time and staff was unable to prevent the injuries. This poses an immediate health and safety risk to children in care.

This requirement is not met as evidenced during the investigation, it was found that a staff member pulled the child from ground from child’s arm, causing injury. This poses an immediate health and safety risk to children in care.

Responsibility for Providing Care and Supervision — Based on observation, the licensee did not comply with the section cited above as LPA observed even with the staff present in classroom, children were constantly engaging in inappropriate actions, which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above as LPA did not observe a current CPR training certificate for any present staff member, which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above as LPA did not observe the TB clearance of each employee on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in three out of six records reviewed today, which poses/posed a potential health, safety or personal rights risk to persons in care.

Any unusual behavior, any injury or signs of illness requiring assessment and/or administration of first aid by staff shall be reported to the child's authorized representative and recorded in the child's record. This requirement is not met as evidenced through staff interviews and record review, that the facility did not reported the child related incidents to the child’s parent. This poses a potential Health and Safety risk to children in care.

Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement is not met as evidenced through LPA's observations. LPA obsreved debris in the play yard and in classroom. This poses a potential Health and Safety risk to children in care.

The notice posted pursuant to subdivision (a) shall remain posted for 30 consecutive days, except that a family day care home shall comply with the posting requirements contained in this subdivision only during the hours when clients are present. This requirement is not met as evidenced through LPA's observations. Upon LPA's inspection, LPA did not observed the previously issued Notice of site visit notice.

The items specified in (d)(1)(A) through (H) above shall also be reported to the child's authorized representative. This requirement is not met as evidenced during the investigation, it was found that a child showed signs of being injured and the facility did not inform the parent of the child in timely manner. This poses a potential Health and Safety risk to children in care.

The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evidenced during the investigation, LPA observed the facility carpets were dirty, trash cans had debry, food carts were dirty. This poses a potential Health and Safety risk to children in care.
