Outdoor Activity Space — Based on observation the licensee did not comply with the section cited above there were three holes in the grassy area of the playground which poses a potential health, safety or personal rights risk to persons in care.
Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on observation and interview, the licensee did not comply with the section cited above where four baby walkers were found on the infant playground. Which poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above where three toxic cleaning products were not locked and accessible to children in the toddler classroom. Which poses a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observation and interview, the facility did not comply with the section cited above where it was observe on the infant playground were four holes in the grass with sitting water, covered with seperated plastic child gate. Additionally throughout the playground, there was also excessive rabbit droppings throughout the grass and concrete pavement. Which poses a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision for Infants — Based on interview record review, the licensee did not comply with the section cited above in ten of ten infant file reviews. It was observed that sleep checks were only recorded earlier in the month and not consistently every fifteen minutes. Which poses a potential health, safety or personal rights risk to persons in care.
