Responsibility for Providing Care and Supervision — Facility Representative (FR) self reported the incident to the department that on 10/27/25, a child was left without adult supervision in the preschool bathroom for approximately 20 minutes, which poses an immediate health, safety, or personal rights risk to persons in care.
Teacher-Child Ratio — LPA record reviews revealed that there were 17 children signed in the facility's parent in and out attendance record and 18 children signed in the facility’s Child Supervision Record (CSR) between 6:00AM and 7:30AM on 09/04/25. CSR shows only one member of staff was signed in from 6AM to 7:30AM with 17-18 children signed in, which poses an immediate health, safety, or personal rights risk to children in care.
