(A)Verification of the completion of the course work required in Section 101215.1(h). A photocopy of a college transcript, or a photocopy of a Child Development Site Supervisor Permit or a Child Development program Director Permit, shall meet this requirement. This was not met as evidenced by: There was a change in director that was never reported to the department. The acting director has been present in her position since January 2026, and this was never reported to the department. This poses a potenital risk to the health an safety of children in care.
(d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone/fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above. In all but one of the preschool classrooms, chemicals were found behind cabinets that were not kept locked. Any harsh chemicals that mention "poison control" on them are considered poisonous and must be kept behind a key lock. This poses/posed a potential health, safety or personal rights risk to persons in care.
