A photocopy of a college transcript, or a photocopy of a Child Development Site Supervisor Permit or a Child Development program Director Permit, shall meet this requirement. This was not met as evidenced by: There was a change in director that was never reported to the department. The acting director has been present in her position since January 2026, and this was never reported to the department. This poses a potenital risk to the health an safety of children in care.
Staff immunization requirements — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review for S1 LPA was not able to review a complete immunization record. The immunization record did not show S1 having Tdap immunization.
