Food Service — Based on interviews and record review, the director did not comply with the section cited above, as children were served food from a bowl that had been contaminated which poses a potential health, safety, and personal rights risk to children in care.

This section is not met as evidenced by: Based on analyst interview and record review, the facility did not comply with the section cited above as they mistakenly fed an infant a bottle with breastmilk that belonged to another infant, which poses a potential health, safety, and personal rights risk to children in care.
Staff-Infant Ratio — Based on LPA interviews conducted, and records reviewed, the facility did not comply with the section cited above as they had a ratio of one (1) staff to five (5) infants, for up to two minutes, on two separate occasions, which poses a potential health, safety, and personal rights risk to children in care.
Personal Rights — Based on analyst interview and record review, the facility did not comply with the section cited above as they mistakenly fed an infant a bottle with breastmilk that belonged to another infant, which poses a potential health, safety, and personal rights risk to children in care.
Staff-Infant Ratio — Based on LPA interviews conducted, and records reviewed, the facility did not comply with the section cited above as they had a ratio of two (2) staff to nine (9) infants, for up to three minutes, on two separate occasions, which poses a potential health, safety, and personal rights risk to children in care.
Plan of Operation — Based on analyst interview and record review, the facility did not comply with the section cited above as they did not follow their own written plan of operation when a child who was determined to have head lice returned to the child care center when they still had signs of the infestation which was a potential risk to other children in care.
Infant Care Food Service — staff is not following feeding plan by not properly documenting feeding times therefore reflecting the feeding schedule to be off. This may pose a threat to the health and safety of children in care.
Food Service — the facility staff feeding marshmallows, uncooked raisins, and hard pretzels to children in care. This may pose a choking hazard and a threat to the health and safety of children in care.
