Child's Records — Based on a file review for child C1 performed on 10/21/25, the facility was unable to provide the LPA with child C1's record during the initial investigation visit. The center director, Anais Heim stated that she was unable to locate the contents of C1's file and that no other records were available for review at the time. This poses a potential risk to persons in care.
