Teacher-Child Ratio — Based on observations, interviews, and record review, on 7/7/26 Staff 1 (S1) who does not presently have the required qualifications and is enrolled in an early childhood education program and Staff 2 (S2) were caring for 17 children which poses a potential risk to the health of the children in care.
Teacher Aide Qualifications and Duties — Based on interview and record review, Staff 1 (S1) does not presently have the required qualifications and is enrolled in an early childhood education program which poses a potential risk to the health of the children in care.
Buildings and Grounds — Based on observations, interviews, and record review, LPA observed rodent droppings in discovery twos preschool room which is licensed but currently not in use. This condition poses a potential health risk to the children in care.
Personnel Requirements — Based on record review, LPAs observed that Staff 6 (S6) had "Heartsaver First Aid CPR AED" which poses a potential health, safety, and personal rights risk to children in care.
Responsibility for Providing Care and Supervision — Based on LPA’s interviews, 6 out of 9 staff stated they witnessed child #1 (C1) left unattended on playground. This action poses an immediate risk to the health and safety to children in care.
