Food Service — C1 was served strawberries on 08/22/2025. C1 is allergic to strawberries which was previously communicated to staff and documented in the latest version of an admissions agreement. This poses an immediate risk to the health and safety and personal rights of children in care.

Food Service — Based on interviews and relevant documents reviewed, C1 and C2 were provided food that they were allergic to or intolerant. This poses an immediate risk to the health, safety and personal rights of children in care.

Personal Rights — Due to interviews and documentation gathered, it was determined that children in care were not assisted during restroom times and children were sent home with dry fecal matter on their bodies, which does not meet hygenic standards and regulations. This poses an immediate risk to the health, safety and personal rights of chidlren in care.

Health-Related Services — LPAs observed a jar of over the counter medicinal ointment on the bathroom floor of the preschool classroom. This poses an immediate risk to the health and safety of children in care.

Teacher-Child Ratio (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance.... This requirement has not been met based on: The records obtained for 07/25/2024 showing an Aide with only 3 credits was left alone with at least 7 children along with interviews.

Personal Rights — Based on complaint allegation evidence collected, the Licensee did not ensure that children were given dignity in their personal relationships with staff as S1 is yelling at children, which poses an immediate risk to the health, safety, and personal rights of children in care.

Teacher-Child Ratio — The Licensee did not ensure that staff are not with more than 12 preschoolers at one time as classroom #3 was observed with twenty four children, staffed with one fully qualified teacher and one qualified aide, which poses an immediate risk to the health, safety, and personal rights of children in care.

Personal Rights — The Licensee did not ensure that the children were given safe and healthful accommondations during meal time, in providing diaper changes, and in maintaining nap mats, which poses an immediate risk to the health, safety, and personal rights of children in care.

Teacher-Child Ratio — The Licensee did not ensure that staff are not with more than 12 preschoolers at one time as a staff member was observed alone in the classroom with 18 preschool children which poses an immediate risk to the health, safety, and personal rights of children in care.

Buildings and Grounds — The Licensee did not ensure that the facility was kept clean, safe, and sanitary as food was left out that children consumed, nap cots were pushed through, and milk/food was drank by multiple children which poses an immediate risk to the health, safety, and personal rights of children in care.

Teacher-Child Ratio — Based on interviews and record reviews, LPA learned that two staff do not meet the minimum requirements for qualifications which places the classrooms out of ratio.This poses an immediate risk to the health, safety and personal rights of children in care.

Teacher Qualifications and Duties — Based on interviews and record reviews, LPA learned that two staff do not meet the minimum requirements for qualifications.This poses an immediate risk to the health, safety and personal rights of children in care.

Responsibility for Providing Care and Supervision — LPA observed preschool age children on the playground without visual supervision of staff. This poses a potential risk to the health, safety and personal rights of children in care.

Criminal Record Clearance — Staff #1 is not Associated to the Guardian Roster for the Facility. This poses a potential risk to the health, safety and personal rights of children in care.

Reporting Requirements — Witness testimony provided to the Department, revealed occurrences where a lack of supervision or attention to children's needs were not addressed due to Staff #1's (S1) inability to complete tasks required of the role. These occurrences were not reported to the Department. This poses a potential risk to the health, safety or personal rights of children in care.

Personal Rights — S1 engaged in aggressive behavior in front of children. This poses a potential risk to the health, safety and personal rights of children in care.

Fixtures, Furniture, Equipment and Supplies — LPA observed classrooms to have a temperature of 54 - 58 degrees in all preschool classrooms. This poses a potential risk to the health, safety and personal rights of children in care.

Notice to parents that site visit reports are available — Based on record review, LPA observed three out of five files missing LIC9224 receipts for reports dated 08/27/2025, 06/19/2025, 06/17/2025 detailing a Type A Citation. This poses a potential risk to the health and safety of children in care.

Responsibility for Providing Care and Supervision — Director stated C1 did not have a diaper change from 10:43 AM - 4:06 PM. This poses a potential risk to the health, safety and personal rights of children in care.

Food Service — meal logs showing a fruit or vegetable component missing from breakfast and snack meal service. This poses a potential risk to the health and safety of children in care.

Responsibility for Providing Care and Supervision: (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This regulation has not been met as evidenced by: Due to LPAs obervations and confidential interviews, it was determined that staff are not visually supervising children in care during restroom breaks along with visual supervision lacking during the outdoor play time due to blind spots in the outdoor activity space. This poses a potential risk to the health, safety and personal rights of children in care.

(1)No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This regulation has not been met as evidenced by: C2 entered classroom with out staff supervision and was alone until S3 located her. This poses a potential threat to the health and safety of children in care.

This regulation has not been met as evidenced by: Reporting Party and Staff witnessed S1 with her back to the children. The children were not visually supervised, this poses a potential risk to the health, safety and personal rights of children in care.

Personal Rights — parent testimony and staff witnessing S1 yelling harshly at children in care. This poses a potential risk to the health, safety and personal rights of children in care.

Notice to parents that site visit reports are available — During today's records review, 7/7 files reviewed did not have the LIC9224 dated 09/18/2024 the date of a report that details a Type A Citation. This poses a potential risk to the health, safety and personal rights of children in care.

Drinking water lead testing — Based on record review, the licensee did not comply with the section cited above in 1 out of 1 times which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in 2 out of 5 staff files which poses a potential health, safety or personal rights risk to persons in care.

Health-Related Services — Based on records review, the licensee did not comply with the section cited above in 3 out of 7 medications which poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — on 1/17/2025 teachers closed the door to the playground leaving child outside. This posed a potential risk to the health and safety of the child in care.

Personal Rights — ABA Therapist testimony of teacher ridiculing child, pulling the child and refusing bathroom priveleges.

Notice to parents that site visit reports are available — During file review, LPAs observed that the Site Director did not obtain signature for the LIC9224 for majority of the children's files, including those newly enrolled, which poses a potential risk to the health, safety, and personal rights of children in care.

Napping Equipment — The staff in classroom #3 did not ensure that each childs bedding did not come into contact with other children's bedding, as four nap blankets/rolls were observed to be stacked on top of the nap cots touching each other, which poses an potential risk to the health, safety, and personal rights of children in care.

Responsibility for Providing Care and Supervision — The staff in classroom #3 did not provide care and supervision to meet the children's needs (per definition (3)(A), (3)(E), (3)(F), (3)(G)), which poses a potential risk to the health, safety, and personal rights of children in care.

Child's Medical Assessments — During review of children's files, LPAs observed that three out of twenty one files did not contain a physicians report (LIC701) prior to enrollment or within thirty days following enrollment, which poses a potential risk to the health, safety, and personal rights of children in care.

Notice to parents that site visit reports are available — During review of classroom #4 files, LPAs observed twenty out of twenty one files did not contain signed verification (LIC9224) of report from 3/14/2024 regarding Type A citations, which poses a potential risk to the health, safety, and personal rights of children in care.

Napping Equipment — The Licensee did not ensure bedding did not come into contact with other children's bedding as bedding was observed to be stacked on a rolling cart, which poses a potential risk to the health, safety, and personal rights of children in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in which Staff 5 did not have a valid Mandated Reported training on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in which Staff 3, 4, and 5, do not have current immunizations and also do not have a current flu vaccine or flu opt out statment on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review and interview, the licensee did not comply with the section cited above Staff 3 does not have a current Health Screening or TB test on file which poses/posed a potential health, safety or personal rights risk to persons in care.
