Child's Records — Based on five children's (C1-C5) records reviewed at 10:06am which revealed C1 & C4-C5's LIC 701 were missing or incomplete. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on statements provided by CD & C1-C3 which confirmed S1 threw or pushed C1 to a sitting position. This posed a potential health, safety and/or personal rights risk to children in care.
Reporting Requirements — Based on the facility not notifying the department of an unusual incident in a timely manner. This posed a potential health, safety and/or personal rights risk to children in care.
Staff immunization requirements — Based on three staff (S1, CD & AD) records reviewed at 12:00pm which revealed CD & AD were missing proof of immunity against Measles. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on five children's records reviewed at 10:05am which revealed C2, C4-C5's Physician's reports (LIC 701) were incomplete. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on LPA's observations of several children were either not signed in or out by their authorized representative. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
