Staff immunization requirements — Based on record review, the facility did not posses record of staff 2's immunization against measles which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the facility did not possess record of Director Justice Willis' health screening which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the mandated reporter training certificates for staff 3 and staff 4 were expired which poses a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on record review, the facility did not possess record of child 3's health history form and needs and services plan which poses a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the facility did not possess record of child 2, child 3, and child 5's complete physician report which poses a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on record review, the facility did not possess complete immunization records for child 3, 4, and 5 which poses a potential health, safety or personal rights risk to persons in care.

Infant Care Discipline — Based on A1, A5, A7 & P1 interviews, S1 placed C1 & C2 in crib as a form of discipline or punishment which poses a potential Health, Safety and Personal Rights risk to children in care.

Responsibility for Providing Care and Supervision for Infants — Based on A1-A2, A5 & A7-A8 interviews which stated they saw C1’s black eye but didn’t know how it happened which poses a potential Health, Safety and Personal Rights risk to children in care.

Health-Related Services — Based on A1-A2, A5 & A7-A8 saw C1 sustained a black eye and licensee did not obtain specific instructions from the authorized representative regarding action to be taken which poses a potential Health, Safety and Personal Rights risk to children in care.

Infant Care Personal Services — Based on CD, CD1, AD & ACD, A1-A5 & A8 interviews and KinderCare app log C1's diaper was not changed as needed throughout the day which poses a potential Health, Safety and Personal Rights risk to children in care.

Responsibility for Providing Care and Supervision for Infants — Based on interviews conducted by LPA, staff are not actively supervising children due to S1-S3 & S5 stated they did not witness C1’s incident that caused injuries and S1’s statements that do not corroborate, which poses a potential Health, Safety and Personal Rights risk to children in care.

Health-Related Services — Based on the LPA’s interviews, it was corroborated that the facility did not notify C1's parent of head injury upon pick-up time the day of the incident. It was corroborated through interviews that C1 obtained a head injury while in care. This posed a potential risk to the health and safety of children in care.

Responsibility for Providing Care and Supervision for Infants — Based on interviews and photographic evidence that staff are not actively supervising children due to being on their cellphone, which poses a potential Health, Safety and Personal Rights risk to children in care.

Observation of the Child — Based on interviews that staff are not reporting incidents that occur to parents or child's authorize representative,which poses a potential Health, Safety and Personal Rights risk to children in care.

Personal Rights — Based on evidence received in which two staff are having an argument, yelling derogatory terms while children are present, which poses a potential Health, Safety and Personal Rights risk to children in care.

Personal Rights — Based on interview with S1 who admitted to aggressively grabbing cup from C1, as a method of handling C1’s undesired behavior, which posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on staff records reviewed at 10:56am which confirmed S1-S5 & S7's staff immunization records were incomplete. The licensee did not comply with the section cited above in for several staff which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on staff records reviewed at 10:56am which revealed S1-S6 were missing Health Screening & S1-S2's records did not contain evidence of negative TB clearances. The licensee did not comply with the section cited above for several staff which poses a potential health, safety or personal rights risk to persons in care.

Teacher Qualifications and Duties — Based on staff record reviewed at 10:56am which revealed that S6's record was missing transcript. The licensee did not comply with the section cited above in count of one out of six staff which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on staff record reviewed at 10:56am which revealed S2 & S4 were missing AB 1207 Mandated Reporter Training certificates. The licensee did not comply with the section cited for several staff which poses a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on six children's (C1-C6) records reviewed at 9:40am which revealed C1-C6's record did not contain date of admission. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Modifications to Infant Needs and Services Plan — Based on six children's (C1-C6) records reviewed at 9:40am which confirmed C1-C6's Infant Needs & Services Plans were not up to date. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Sign In and Sign Out — Based on LPAs' review of the children's sign/out procedures which appeared to be incomplete. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
