Observation of the Child — … Based upon interviews with parent and staff members, and medical records received, C1 fractured her elbow during morning play, and C1 was not continually observed throughout the day, nor parent contacted by staff members which posed an immediate health, safety and personal rights risk to children in care.

Family home — in-ground pool drowning prevention requirements — Based on record review, the licensee did not comply with the section cited above, as the facility did not maintain a pool inspection log for review by the Department, which poses a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on LPA interviews, the toileting and/or diapering needs for child C1 were not met, which posed a potential health, safety or personal rights risk to children in care.

Observation of the Child — Based on LPA interviews, the behaviors and actions of child C1 were not observed by staff during outside playtime, resulting in the child being partially unclothed and exposed, which posed a potential health, safety or personal rights risk to children in care.

Disaster and Mass Casualty Plan — THIS IS NOT A DEFICIENCY. FACILITY CONDUCTED A FIRE DRILL ON SEPTEMBER 2024.

Responsibility for Providing Care and Supervision — Based on interview and record review, the licensee did not comply with the section cited above. Staff failed to provide visual supervision on 11/15/2024 during transition, a 2 year old child was left in the classroom while the rest of the children transitioned to the outdoor play area. According to staff, child was without visual supervision for approximately 2 minutes. This posesd a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above. Two staff missing Mandated Reporter Training on file. This poses a potential health, safety or personal rights risk to persons in care.

Teacher Qualifications and Duties — Based on record review, the licensee did not comply with the section cited above. Staff T.H. is missing a copy of transcript on file. This poses a potential health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — There were 13 children from the Preschool Classroom that were at the playground supervised by staff Lauren Digiacinto. Staff thought that she only had 12 children. This poses a potential safety risk to children in care.

Criminal Record Clearance All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement is not met as staff Andrehya Azzato has been working at the facility for over a year and her fingerprint is not associated to the facility. This poses a potential health and safety risk to children in care.
